NOTICE - TERMS OF USE

 

The Client (identified in the signature block below) is hereby notified and acknowledges that on the date scheduled, he/she/they have scheduled a service at The Center Origin, 1440 Blake Street, Unit 330, Denver, CO 80202.

The Client is hereby notified of and agrees and acknowledges that:

• Service Provider: The Services will be provided by the Provider selected.

 

• Location Ownership/Operation: The Provider will perform the Services at the Location, which is owned/leased or operated by The Center Origin, LLC.

 

• Provider Independence: The Provider is an independent contractor, not an owner, representative, or agent of the Service Company. The Provider operates autonomously without the ability to bind the Service Company in any capacity, and without influence from the Service Company. 

 

• Billing Responsibility: The Service Company will handle booking, billing, and collection directly from the Client for the Services.

 

• Scope of Services: The Service Company’s services are limited to those explicitly outlined in this Notice.

 

• Liability: THE CLIENT AGREES TO HOLD HARMLESS THE SERVICE COMPANY, ITS OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, AND REPRESENTATIVES FROM ALL CLAIMS, LIABILITIES, DAMAGES, LOSSES, COSTS, AND EXPENSES ARISING FROM THE SERVICES, EXCLUDING CLAIMS RELATED TO PAYMENT. THE CLIENT ASSUMES ALL RISKS AND RELEASES THE SERVICE COMPANY FROM ANY LIABILITY RESULTING FROM THE SERVICES UNLESS SPECIFICALLY ADDRESSED OTHERWISE.

 

Client’s Acknowledgment: I HAVE READ AND UNDERSTOOD THE CONTENTS OF THIS NOTICE AND BY SUBMITTING PAYMENT AND BOOKING THE APPOINTMENT, I AGREE TO ITS TERMS.
